Pefakit® PiCT®

Pefakit® PiCT® (Prothrombinase induced Clotting Time) is a plasma based functional assay for the quantitative determination of anticoagulant activities of heparin and heparinoids based on FXa and FIIa inhibition.
For PiCT® analysis the plasma sample is mixed with a reagent containing a combination of a defined amount of activated factor X (FXa), phospholipids and RVV-V, an enzyme from the venom of the snake Daboia russelli specifically activating factor V. During an incubation period of 180 s the added FXa is inhibited depending on the amount of AT-heparin complexes or direct FXa and FIIa inhibitors present in the sample. Following recalcification the prothrombinase complex is formed using the residual FXa, phospholipids, factor Va generated from factor V present in the sample and free calcium ions. Due to the factor V preactivation prothrombinase formation is not dependent on endogenous thrombin mediated factor V activation. Therefore the new assay has been named "Prothrombinase-induced Clotting Time" (PiCT®). The time until the detection of clotting is recorded.

The Pefakit® in-TDT® (TDT, Thrombin Dynamics Test; in, intrinsic pathway) allows a kinetic determination of thrombin formation in platelet-poor plasma by activation of the intrinsic coagulation pathway.
Coagulation is triggered using a selected contact activator reagent. Thrombin formation is detected with a chromogenic substrate being readily converted by thrombin and releasing p-nitro-aniline (p-NA), which is continuously recorded at 405 nm. Fibrin gelation in the sample is inhibited by the addition of a fibrin polymerization inhibitor, hence allowing an undisturbed optical detection. The first derivative of the resulting curve is calculated, representing the velocity of the conversion of the chromogenic substrate. For test principle see also the cartoon in the appendix.
Two parameters are determined: the thrombin onset time delineates the time point when free thrombin is detected (begin of increasing optical density during the reaction). The second parameter is the peak value of the first derivative that represents the maximum velocity of conversion of the thrombin substrate and thereby characterizes the dynamics of thrombin formation. This parameter is defined as thrombin dynamics and is usually expressed as % of normal.
